Pastoral Care Emergency Contacts, Buddies, Life Events and Memorial Wishes

Pastoral Care would be grateful for information to help us support you (AFM members and attenders) in times of need and recognize life's celebrations.  Is there information you would like Pastoral Care to have on hand if you have an emergency or were to pass away suddenly? Would you welcome recognition from the Meeting on important life events?  Would you like to be paired with a "Buddy" to check in for safety?  Please provide the information below as is relevant for you.  If you are not comfortable or do not wish to answer any of these questions, please leave them blank.

If you are unable to make decisions for yourself, is there someone we should contact who is responsible for medical, legal, or long-term caree decisions?

In case you are hospitalized or otherwise unable to come home and you need help with your house, does someone have keys to house / apartment or are they hidden on the property?

BUDDY: Those who live alone may wish to be paired with another Annapolis Friend to check in with each other regularly. Let us know if you would like to have a Buddy or be a Buddy—or both (check all that apply).

LIFE EVENTS: We would like to be able to celebrate with you on happy occasions and support you during challenging events. If you would like us to reach out to you on special occasions (birthdays, anniversaries, etc.) please provide the following information:

Would you like your name on the group metal plaque and/or a bluestone marker?
